2013 Season Registration Information

Regular registration begins April 1st to May 31st. Registration fees are $130 per player through May 31st and $155 per player after May 31st. Discounts are available for families with three or more siblings.

$50 fifth sibling

After May 31st
$155 per child

Parent/Guardian needs to provide:

  1. a completed and signed CYSA Membership Form (pdf). NOTE: This form must be signed in either blue or red ink. No photocopies will be accepted.
  2. a completed and signed Parent Sideline Ethics Agreement (pdf). 
  3. a copy of birth certificate for new players only (hospital/baptism certificate not accepted). Returning players do not need to submit a copy of their birth certificate.
  4. a 1.5" x 1.5" picture (U9 and above). If your child took a picture at last year's jamboree, picture is not necessary.
  5. payment for appropriate amount (checks payable to EGYSL).
  6. a completed and signed Special Request Form for players playing up an age group.

Late Registration 

Beginning June 2, late registration fees are in effect. Registration fees are an additional $25 per player. Late registration fees are as follows.

Beginning June 1
$155 first or second sibling
$125 third sibling
$105 fourth sibling
$75 fifth sibling
