Players/Managers- Use this form to provide us with feedback on how your official performed in your game.

Our emphasis for our officials is #1- make the game fun, #2- keep the participants safe, #3- be approachable and knowledgeable.  Please keep this in mind when you are providing feedback.  Comments like "your umpire was horrible" doesn't identify the three areas we work with our officials on.  Comments like "We had an issue with a basketball official allowing too much contact between players, which is a safety concern." can help our agency work with that employee to improve their performance.

Please include the sport you play, the night you play and the league you are playing in.

When rating officials, 3 is excellent, 2 is satisfactory, and 1 is unsatisfactory.
