Volleyball Court Waiver
I, ___________________________, acknowledge that this activity entails risks or injury to myself and a risk or injury to spectators or other third parties as a result of my actions. I agree, covenant and promise to accept and assume all responsibility and risk for injury, death, illness, disease, or damage to spectators or other third parties and their property arising from my participation in this activity. I agree to release Forest View Lanes, The Bar at Forest View Lanes and Sandy Courts, from any and all liability
My signature on this wavier is my acceptance and is verification (in of itself) that I agree to the above terms.
