All game-day staff must have the following forms on file in order to be paid for work.

Independent Contractor Form

Youth Work Permit U16 - Youth Work Permit - For workers under the age of 16

Youth Work Permit 16-17 - Youth Work Permit - For workers that are 16 or 17 years old

**No Work Permit required for workers 18 or older
